Services Provided
Veritas Behavioral Group provides clinical behavioral health evaluations to determine whether an emotional support animal is appropriate as part of a treatment plan. These evaluations are conducted by licensed behavioral health professionals. We do not provide ongoing therapy, medication management, crisis intervention, or any other clinical services beyond the scope of the evaluation.
No Guarantee of Documentation
By submitting an intake form and payment, you acknowledge that documentation is not guaranteed. All outcomes are determined solely by the licensed provider assigned to your evaluation, based on their independent professional clinical judgment. The fee covers the clinical evaluation, not the issuance of documentation.
Refund Policy
If the assigned licensed provider determines that documentation is not clinically appropriate based on the information provided, you may be eligible for a refund of the evaluation fee. Refund eligibility is subject to review. No refund is available once documentation has been issued. Please contact us to initiate a refund request.
Eligibility
Services are available to individuals who are 18 years of age or older, reside in a state currently served by Veritas Behavioral Group, and are seeking an evaluation for themselves (not on behalf of another individual). You must provide accurate and complete information during the intake process.
Accuracy of Information
You agree to provide truthful, accurate, and complete information during the intake and evaluation process. Providing false or misleading information may result in cancellation of your evaluation without refund and may have legal consequences.
No Ongoing Provider Relationship
Completion of an evaluation with Veritas Behavioral Group does not establish an ongoing patient-provider relationship. The evaluation is a limited-scope clinical service. For ongoing mental health treatment, therapy, or medication management, please consult with your primary care provider or a licensed mental health professional.
Use of Documentation
Documentation issued through Veritas Behavioral Group is intended for use in housing-related reasonable accommodation requests under the Fair Housing Act. You are responsible for understanding the applicable laws and requirements in your jurisdiction. We do not provide legal advice, and our documentation does not guarantee any housing outcome.
Intellectual Property
All content on this website, including text, design, graphics, and evaluation materials, is the property of Veritas Behavioral Group and may not be reproduced, distributed, or used without written permission.
Limitation of Liability
Veritas Behavioral Group's total liability for any claim arising from the use of our services is limited to the fees paid for the evaluation. We are not liable for any consequential, indirect, or incidental damages, or for outcomes related to housing decisions, legal proceedings, or any other matters arising from the evaluation.
Governing Law
These terms are governed by applicable federal law and the laws of the state in which the provider is licensed. Any disputes shall be resolved through binding arbitration.
Changes to Terms
We may update these terms from time to time. Continued use of our services after changes constitutes acceptance of the updated terms. Material changes will be communicated on this page.
