Veritas Behavioral Group, LLC is a Wyoming limited liability company (EIN 41-5318174), an entity of Nomi Beach Health. VBG provides licensed, professional behavioral-health evaluations conducted by clinicians licensed in the patient's state of residence. Your $99 fee compensates the clinician for the time and professional judgment required to conduct your evaluation, including reviewing your intake, administering standardized clinical assessments (including the GAD-7 and PHQ-9), reviewing the clinical information and documentation you submit, and rendering a clinical opinion.
All evaluation fees are non-refundable.
The fee is for the professional evaluation, not for any particular outcome. Whether documentation supporting an Emotional Support Animal accommodation is issued is determined solely by the licensed clinician's independent professional judgment, applied to your individual clinical presentation and the prevailing standards of care. No outcome is promised, guaranteed, or implied at the time of payment, and your fee is not contingent on any specific result.
By completing payment, you acknowledge and agree that:
- The $99 evaluation fee is earned by Veritas Behavioral Group at the time of payment and is non-refundable in whole or in part.
- No refund will be issued if the clinician determines that supporting documentation is not clinically appropriate.
- No refund will be issued if you change your mind, elect not to complete the clinical evaluation form, or do not return to complete the process after payment.
- No refund will be issued based on disagreement with the clinician's professional opinion.
Billing corrections
If you were charged in error -- for example, charged more than once for a single evaluation -- email esa@vbgesa.com within thirty (30) days. We will correct duplicate or erroneous charges promptly. This is a billing correction, not a refund, and is not conditioned on the outcome of any evaluation.
Chargebacks
If you initiate a chargeback with your card issuer, Veritas Behavioral Group will respond to the dispute with documentation of the professional services ordered and performed. Veritas Behavioral Group reserves the right to pursue collection of evaluation fees for services rendered and to recover reasonable costs associated with defending disputes.
