Complete Secure Intake

Begin by completing our secure intake form with your medical and behavioral health history. The form takes approximately 10 minutes and covers your current symptoms, treatment history, and how they affect your daily functioning.

All information is encrypted and stored in compliance with HIPAA guidelines.

Clinical Provider Review

A licensed behavioral health provider reviews your submitted information in detail. This review assesses the clinical appropriateness of an emotional support animal as part of your treatment plan, based on your specific condition and circumstances.

Providers may follow up with additional questions as needed.

Clinical Evaluation

Your assigned licensed provider conducts a thorough clinical evaluation. This may include a telehealth consultation or asynchronous review depending on your state and clinical requirements.

The evaluation follows evidence-based clinical standards.

Documentation Issued

If the evaluation determines that an emotional support animal is clinically appropriate as part of your treatment plan, documentation is issued by your licensed provider based on their professional judgment.

Documentation is only issued when medically appropriate. Outcomes are not guaranteed.
